Our value-based clinical programs include Choose Control specifically designed for patients with diabetes mellitus type two, ClearWay specifically designed for patients with COPD or other chronic lung diseases, Ventricare specifically designed for patients with congestive heart failure, and MendTrac specifically designed for patients suffering from persistent wounds.
