Supervise direct care staff and coordinate the day-to-day activities and maintenance of the assigned home(s). Maintain communication flow within the Agency and Supported Living Services division. The overall objectives are to provide a home environment that is as normal as possible and to foster the development of each consumer to their highest independence. Supervise direct care staff to ensure the ISP is being implemented for each persons served. Train staff in all programs, using lecture, role modeling, text, and other materials. Coordinate the delivery of training and rehabilitation services. Monitor implementation, and follow-up each consumers ISP. Assist in coordinating the scheduling of CDD's involved in assigned houses. Must be flexible in covering designated shifts on an emergency basis. Assist in overseeing file maintenance and record keeping in the home (bills, receipts, med errors, write ups, goals, 90-day paperwork, behavior data, etc.) Assist in maintaining a weekly cleaning checklist, grocery inventory, and med count. Participate directly in the recreational, therapeutic, and training activities of consumers. Serve as a liaison between consumers, families, staff, professionals, and the community. Safeguard consumers' and families' human and civil rights. Maintain medications ensuring adequate supplies are being maintained. Assist persons served in budgeting their funds with assistance from FEQAA. Maintain adequate supply of all household items. Accurately maintain petty cash through petty cash logs. Formally and informally assist persons served with programming needs. Provide services in compliance with BQIS and BDDSregulations. Actively involve the persons served in community events. Display patience in all situations involving persons receiving services,staff members, parents, and others in the community. Speak in an even, positive and personal tone of voice when conversing with persons served. Must be capable of meeting the physical demands required by caring for the consumers. Physical requirements may include, but not be limited to: bending; reaching; stretching; lifting 25-50 pounds (depending on person served) without assistance; grasping; visual acuity; running; walking; squatting, guiding; transporting; assisting with physical transfer; and manual dexterity to handle medications; and handling physical aggression as defined in CPI training. Ensure that employees follow all departmental and agency policies as well as state and federal laws. Respond to calls as needed as consumer issues arise. Ensure a safe and sound environment for staff and persons served. Assist house manager with on-call duties and all that entails.
B. S. degree in a human service field preferred. High School Diploma or equivalent required plus at least one year of experience with the MR/DD population, preferably in a residential setting. Possess and maintain a clear criminal and driving record. Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier. Must possess and maintain reliable transportation that meets state requirements for transporting persons served. Must be capable of obtaining and maintaining certification in CPR, First Aide, and CPI and perform if necessary, and Medication Administration. Attend meetings and training as requested. Become active in an organization of related professionals.
